What the inspector wrote

On 02/24/2026, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Caregiver, Elizabeth Celso. Administrator, Katie Eiseman was contacted and arrived later in the visit. The facility currently provides care for 10 residents, and 3 staff. Facility is licensed for ages 60 and over. 6 residents may be non-ambulatory. There is a hospice waiver on file for 4 hospice residents. Recently a verbal approval for 5 hospice residents was granted. As of today's visit, there are 5 residents on hospice. This is a two level facility. Residents are mixed between ambulatory and non-ambulatory residents. LPA toured the facility with staff, facility is found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. 5 fire extinguishers are located through out the facility on each floor. LPA observed kitchen, garage and laundry room were found to have extinguishers and are observed to be charged. Carbon monoxide detector are located at each hallway. Smoke and sprinkler systems are interconnected with a fire safety inspection conducted by a private company and agency. Fire panel is observed adjacent to the front door with an inspection date of 02/2026. LPA observed 4 fire pull stations through out the facility. All resident restrooms showers are equipped with non-skid flooring or mats and grab bars for accessibility. Continued on next page... Page 2 There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen and garage, sufficient for residents in care. Food supply is replenished weekly and stored properly. Facility provides a wide variety of meal preferences and preparation while also ensuring proper dietary restrictions are followed. Cleaning supplies and other toxins are safely stored in locked closets in the laundry room, all of which were secured upon inspection. There was a supply of hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings and bedding items. Kitchen has a pin pad lock for security purposes. Food supplies and appliances in the kitchen are in working order. A spot check of medications was conducted and found that all medication counts and records are in order. LPA conducted a sample file review for residents and found all items to be in order including medical assessments and needs & service plans. Upon a spot check of staff files, LPA found that caregiver staff have annual training and current 1st aid and CPR on file and current. Last disaster drill conducted was in December 2025 addressing active shooter situation. Next drill will be conducted in March 2026 per Katie. Administrator certificate is observed to be current expiring 01/18/2027 for Katie Eiseman. LPA requested the following documents be sent to CCL by COB 3/5/2026: LIC 308 Designated Facility Responsibility Liability Insurance No deficiencies cited during today's visit. Report is reviewed with Katie and a copy is provided.
