On 2/5/2025, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Administrator, Alice Zhang. The facility is licensed to provide care for a total of 6 residents all of which may be non-ambulatory. The facility does not have any residents admitted at this time. The Administrator indicated that they are still determining whether they will be admitting new residents and will notify licensing. LPA continued with a tour of the facility with staff, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, restrooms and common areas were inspected. Fire Extinguishers located throughout the facility were found to be charged. Smoke detectors were present in each common room and resident bedroom. The home is currently utilized as a personal living space for the Administrator. Resident's bedroom have lighting & appropriate furnishings and bedding items. Restrooms for resident use were equipped with grab bars and kept in good condition. There is a large outdoor space in the backyard for resident use, equipped with ramp for mobility. Administrator, Alice Zhang's Certification 7006527740 is valid through 12/12/2025. Additionally, LPA informed Administrator of due annual licensing fees. No deficiencies cited during today's visit.
