On 03/10/2026, Licensing Program Analyst (LPA) Yi Sam Jian conducted an unannounced Case Management visit at the facility. LPA met with staff Ethel Atendido(S4) and Roberto Molabola(S3). The Administrator, Maria Jasmin Nelson, was contacted by phone and the purpose of the visit was explained. Administrator stated that S4 was the staff in charge and granted permission for S4 to sign required documents on behalf of the facility. The purpose of the visit was for LPA to deliver an Immediate Exclusion Letter for staff member S1 . Administrator confirmed that S1 is no longer employed at the facility as of this date . The exclusion letter was provided to staff in charge, S4 . This report was reviewed and discussed with staff in charge, and a copy was provided.
