On 11/4/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced visit to follow-up on a plan of correction, LPA met with caregiver, Rosema Caspillan and explained the purpose of today's visit. During the annual inspection on 10/7/2025, the facility received 2 B citations: - Other Provision 1569.625(b)(2) -Reappraisals 87463(a) The plan of correction for the above citations were due on 10/21/2025. However, the administrator did not submit the plan correction until 11/3/2025. The administrator stated that the delay of submission was due to computer issues but the administrator did not take the initiative to contact CCL rather LPA called to administrator on 10/22/2025 and 10/30/2025 to follow-up. During today's visit, LPA spoke with the administrator over the phone and reminded the administrator of the importance of being compliance with POC due date and if failure to submit plan of correction in the future may result in civil penalty. During today's visit, LPA reviewed and cleared the above citations. This report is reviewed and discussed with the caregiver.
