What the inspector wrote

On 12/18/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to deliver conclusionary findings for this complaint received by the Department on 10/14/2025. LPA Calandra was greeted by Chris Schuster, Administrator and explained the purpose of the visit. Complaint alleged that staff did not administer medications to a resident in care. When R1 moved into the facility, a list of medications was provided to the Licensee. Based on interviews and record review, the Licensee only received two of the three pages of the Centrally Stored Medication records from R1's provider. Thus, the medications were not provided to R1. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. An exit interview was conducted. This report was reviewed with facility representative and a copy provided.
