What the inspector wrote

On October 11, 2022, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced case management visit to deliver the findings for the above allegation. LPA met with Administrator, Mindy Han and Regional Business Development, Jeffrey Dillon and explained the purpose of the visit. During the investigation of complaint control number; 14-AS-20211221110610, LPA Charitra reviewed Resident #1's (R1's) file and found R1's physician's report was dated 1/19/2018. The facility failed to keep an updated physician's report for R1 to document R1's change of condition. Deficiency of the Residential Care Elderly California Code of Regulations, Title 22, Division 6 is observed and cited on a LIC 809D. Failure to correct the deficiencies may result in civil penalties. Report is reviewed with Administrator, Mindy Han and Regional Business Development, Jeffrey Dillon and a copy is provided with appeals rights.
