Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.Based on records review, this facility wasfound to be deficient of this section as evidenced by the presence of bed bugs from a contracted pest control assessment and report conducted on 09/26/2024. This presented an immediate threat to the Health, Safety, and Personal Rights of the residents in care.

This requirement is not met as evidenced by: Based on interview and record reviews, Licensee did not ensure the timely delivery of a nutritional supplement for R1 as ordered by Physician on 10-8-21 and 10-19-21. Nutritional supplement was given to R1 on 10-23-21. This poses a potential health and safety risk to residents in care.

Based on LPA observation, the licensee did not comply with the section cited above in 1569.311. LPA observed one missing carbon monoxide which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, the licensee did not comply with the section cited above in 87303(a). LPA observed front door and emergency door in disrepair and blinds in shared bedrooms in disrepair, which poses/posed a potential health, safety or personal rights risk to persons in care.
