Unannounced case management visit made out to this facility on 10/13/2021 by LPA Charlie Yang. LPA Yang was met by the facility designated Administrator, Tracy Freudenthal, and another member from the California Department of Public Health. The purpose of this visit was to tour the facility and review the physical plant in response to the recent increase in COVID related resident and personnel cases. Tour of the facility was conducted. Exit Interview was conducted by Kristy Trausch, California Department of Public Health, with tips and recommendations related to reducing the number of COVID cases. A copy of this report will be scanned and sent over to the facility designated Administrator, Tracy Freudenthal, due to COVID related concerns while at this facility. As a result, Tracy Freudenthal will have to sign the scanned copy and resubmit back to this LPA. Administrator signature is on file.
