Correction check·November 18, 2022

Unannounced plan of correction visit made out to this facility on 11/18/2022 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator Sheryl Bravo who was briefly interviewed. Current census was 59 residents. The purpose of this visit was to review the plan of corrections that were due from prior annual visit that was conducted on 08/04/2022. It was learned that a work order had been completed and approved for construction to demolish the fence that had fallen down and a new one to be put up. The work order that was commissioned was finally approved on 09/23/2022 in the amount of $5,345.00. Construction was slated to begin soon and proof of the completed work will be submitted into CCL for review by this LPA. There were no deficiencies observed or cited during today's Plan of Correction visit. Exit Interview
