Licensing Program Analyst (LPA) Ashley Boothe arrived unannounced on 10/21/2021 at 10:45am to an unannounced case management visit following an incident report submitted to the department. LPA met with Administrator and stated the purpose of the visit. Current census is 69. During the visit LPA conducted interviews and reviewed records. LPA reviewed record of one incident where Resident one (R1) was observed by staff one (S1) showing symptoms of primary diagnosis as updated on most recent physician's report. Upon return to facility R1 was offered increased level of care due to change in condition and R1's family moved R1 out the same day to avoid increased in monthly fees. Per the California Code of Regulations, Title 22, Division 6, no deficiencies observed or cited. Exit interview held, copy of report given.
