Fire extinguisher was observed by House manager and LPA to have tagged dates of 05/24/2021.

87616 Exceptions for Health Conditions (a) As specified in Section 87209, Program Flexibility, the licensee may submit a written exception request if he/she agrees that the resident has a prohibited and/or restrictive health condition but believes that the intent of the law can be met through alternative means.(b) Written requests shall include, but are not limited to, the following:(1) Documentation of the resident's current health condition including updated medical reports, other documentation of the current health, prognosis, and expected duration of condition.(2) The licensee's plan for ensuring that the resident's health related needs can be met by the facility.(3) Plan for minimizing the impact on other residents. R1 is being provided with home health services after graduating from hospice on 3/3/2022.

This requirement is not met as evidenced by records review the facility does not have records of home health services to address the needs of R1. This poses an immediate risk to residents in care.

87465 Incidental Medical and Dental Care (h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement was not met as evidenced by: observation. Licensee failed to ensure prescription and non prescription medicated were inaccessible to residents. LPA observed Influenza vaccine and four medication for R1 to be accessible to residents in the main refrigerator. This poses an immediate health and safety risk to residents in care.
