What the inspector wrote

On 8/17/2021 at 10:05am Licensing Program Analyst (LPA Ashley Boothe conducted a case management visit to the facility for the purpose of observing Person one (P1) is no longer working at the facility due to exclusion order. Administrator stated P1 has not been at the facility since P1's last day worked on 4/28/2021. Facility terminated P1 on 4/29/2021. LPA explained P1 cannot be allowed to work, be present and/or live in a CCL licensed facility or have contact with clients in any residential facility or child day care licensed by the California Department of Social Services. Exit interview conducted, a copy of this report provided on this date. A signature on these forms acknowledges receipt of these forms.
