The LPA met with Nadya Rosales The following deficiencies, initially cited during a visit on 08/20/2025, have been cleared: Section Cited: 87465(d)(1) Date Due: 08/21/2025 Plan of Correction: Facility Administrator will review the section, 87465(d)(1). A statement of correction, along with proof of staff training for no less than (1) hour in duration, for the cited section will be completed and submitted to the LPA’s email by the due date of 8/21/2025 COB at 5:00pm.
