What the inspector wrote

On 6-4-2025 at 10:15am Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a case management visit regarding Licensee’s procedure of converting sole proprietorship to Limited Liability Corporation (LLC). LPA met with Licensee/Administrator Virginia Yadao and explained the purpose of the visit. LPA conducted brief interview with Licensee. On 5-22-25, Licensee/Administrator informed Regional Office of a facility change to LLC. It was determined that Licensee/Administrator did not comply with applicable laws and regulations including submission of a change of ownership application prior to the conversion resulting in a forfeiture of license, and further did not comply with applicable regional center procedures. LPA informed Licensee/Administrator that a new application for change of ownership must be submitted to the Department’s Centralized Application’s Bureau (CAB) no later than June 13, 2025 to avoid further action including but not limited to additional citations and civil penalties. LPA referenced Health and Safety Code Section 1569.19 and discussed with Licensee/Administrator during today’s case management. As a result of today’s case management, citation is issued under Title 22, Division 6 and noted on LIC 809D. An exit interview was conducted with Licensee/Administrator and a copy of this report was provided. Appeal rights provided.

This requirement was not met as evidenced by: Based on interview, Licensee/Administrator did not comply with regulatory procedures and applicable regional center procedures for a change of ownership of facility. This poses an immediate health and safety risk to residents in care.
