What the inspector wrote

Unannounced case management visit made out to this facility on 03/30/2026 by this Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility representative, Rachel Rae Santiago, who was briefly interviewed and was requested by this LPA to inform the facility designated Administrator, Rolando Santiago, that CCL was present at this time. The facility designated Administrator, Rolando Santiago, was present and briefly interviewed at this time. Current census was 3 residents. The purpose of this case management visit was to follow up on some potential issues that were recently brought to the attention of this LPA at this time: Wood Flooring in need of repair/replacement Tiles in common areas in need of repair/replacement Missing window screens-others in need of repair/replacement Facility perimeter fence The following deficiencies were observed and cited on the following LIC 809-D pursuant to Title 22 Rules and Regulations, Health and Safety Codes. Appeal Rights were printed and a version was given to the facility designated Administrator at this time. Exit Interview

Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.This facility was found to be deficient as evidenced by the perimeter fence leaning and in need of repair/replacement which poses a potential risk to the Health, Safety, and Personal Rights to all residents in care at this time.

    All window screens shall be clean and maintained in good repair.This facility was found to be deficient as evidenced by the lack of window screens in the facility windows which posed a potential risk to the Health, Safety, and Personal Rights to all residents in care.

    The following space and safety provisions shall apply to all facilities: The premises shall be maintained in a state of good repair and shall provide a safe and healthful environment.This facility was found to be deficient as evidenced by the observation of the wood flooring in the common areas that was in need of repair/replacement at this time. In addition, the tiles in the facility restrooms were cracked and/or broken in need of repair/replacement at this time which posed a potential threat to the Health, Safety, and Personal Rights of all residents in care.
