Correction check·February 4, 2026

Unannounced Plan of Correction visit made out to this facility on 02/04/2026 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated representative Daisy Flores. A brief interview was conducted with the facility designated representative at this time. Current census was 5 residents. The purpose of this visit was to follow up on the deficiencies that were cited from a prior complaint visit conducted on 01/21/2026. This visit was to follow up on the Plan of Correction that was due. The following deficiency was observed and cited on 01/21/2026: Living accommodations and grounds shall be related to the facility's function. The facility shall be large enough to provide comfortable living accommodations and privacy for the residents, staff, and others who may reside in the facility. No room commonly used for other purposes shall be used as a sleeping room for any resident. This includes any hall, stairway, unfinished attic, garage storage area, shed or similar detached building. Plan of Correction clearance letter was printed and a copy was provided to the facility designated representative at this time. There were no further deficiencies observed or cited during today's Plan of Correction visit. Exit Interview
