What the inspector wrote

Unannounced Case Management visit made out to this facility on 04/28/2025 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility designated Administrator Andrea Armstrong. A brief interview was conducted with the facility designated Administrator at this time. Current census was 93 residents.. The purpose of this visit was to follow up on a report of a facility resident requiring care and supervision related to a Restricted Health Condition. This visit was conducted to make sure that there was a facility care plan set into place with the appropriate level(s) of care and trained individuals involved within this care plan at this time. This review was conducted in order to be able determine if this facility was able to provide and assist with the proper level of care to this resident, R1, at this time. A review of the facility care plan was conducted for R1. There were no deficiencies observed or cited related to this case management visit at this time. Exit Interview

The facility was found to be deficient as evidenced by the presence of incomplete forms and documents for staff assignments not being properly filled out and initialed when tasks for proper care and supervision to the residents were completed which posed an immediate threat to the Health, Safety, and Personal Rights of residents in care.
