What the inspector wrote

Licensing Program Analyst (LPA) Kesha Lewis arrived unannounced to conduct a case management visit regarding complaint 27-AS-20250204143321. LPA Lewis met with facility administrator FLORES, OLIVER and explained the purpose of the visit. LPA Lewis had Substantiated complaint 27-AS-20250204143321 and the plan of correction was to issue a refund. Mr. Flores reached out to LPA Lewis and asked for an extension because he was going to file an appeal. As of todays date no appeal has been filed and no refund has been issued. LPA Lewis is reissuing the citation on todays date. Citations are issued as indicated on LIC 809D. An exit interview was conducted, Appeal rights provided.
