On 05/05/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Ebony Aliriagwu and explained the purpose of the visit. There are currently no residents in care at this facility. LPA Pascua verified that FDA Aliriagwu has an active certificate #7036602740 and expires on 03/17/2028. A tour of the facility was conducted to ensure that facility safety was in compliance, such as the fire extinguishers, smoke detectors, and carbon monoxide were present. All were present and in good repair at this time. A tour of facility bedrooms were conducted to verify that there are currently no residents in care. LPA advised FDA Aliriagwu to notify the department once facility accepts residents in care. Based on the observations made, there are no deficiencies observed during this annual visit. An exit interview was conducted and a copy of this report was provided to the facility.
