On 12/09/25, Licensing Program Analyst (LPA) Kesha Lewis made an unannounced visit to this facility to open an investigation into the above allegation. LPA identified herself upon arrival, stated the purpose of the visit. LPA met with Angela Ringu and a brief interview followed. LPA requested a copy of the facilities activities Calander for November and December, and the LPA requested the name of contract for the activity’s director. Based on documents reviewed there is an activity's director on staff and both months of November and December have different activities for the residents to participate in. Therefore, the allegation is UNFOUNDED. This agency has investigated the complaint allegation(s). This agency has found that the complaint was UNFOUNDED, meaning that the allegation(s) were false, could not have happened and/or was without a reasonable basis. This agency has therefore dismissed the complaint. There were no deficiencies observed or cited during today’s complaint visit. Exit Interview.
