What the inspector wrote

Licensing Program Analyst (LPA) Albert Johnson conduct a case management visit regarding an eviction letters sent to the department. R1 is at risk of eviction. Admitted since February 26, 2025, R1 is currently facing financial difficulties and can only afford to pay $1,700 per month since July 2025, whereas the agreed-upon monthly Board and Care fee is $4,000. R1’s family is grappling with financial challenges and are unable to provide additional support at this time. The facility has contacted Mastercare to explore options for assisting R1 with monthly expenses and ensuring continued care. During the inspection today LPA observed that S1 was not associated to this facility and does not have a file for this facility. Deficiencies cited today on the attached 809D page. An exit interview was conducted.
