What the inspector wrote

Unannounced case management visit made out to this facility on 07/16/2025 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Edgar Parra, and was briefly interviewed at this time. Current census was 81 residents. The purpose of today's case management visit was to follow up on an incident hat was recently submitted into CCL. The following LIC 624 was reviewed and discussed for the following resident that was in care: R1 It was learned that the above referenced resident, R1, had left this facility without properly notifying any staff members and was found to be missing during breakfast time one day. It was learned that R1 was residing in the memory care unit at this time. It was learned that a friend of R1 had come to this facility, under the pretense of taking R1 out for a walk, and was able to get R1 out of the memory care unit. Upon exiting the memory care unit, R1 and this friend got into a car and left this facility without the proper notification to the facility staff or anyone else. A missing person's report was filed with Manteca PD and R1 was later discovered heading towards southern California since it was learned that R1 used to be a resident from that area. The responsible party for R1, who also lived in southern California, decided it was best to keep R1 close to this area and placed R1 in that area. There were no deficiencies observed or cited during today's case management visit. Exit Interview
