Unannounced case management visit made out to this facility on 01/23/2026 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated representative, Charissa Reyes, who was briefly interviewed at this time. Current census was 0 residents. The purpose of this case management visit was to tour this facility and retrieve the license off of the wall since this facility, and it's representative, has decided to close this facility and relinquish the facility license at this time. A tour of the facility was conducted. After further consideration, the facility designated representative decided that she was going to rescind the decision to close this facility and wanted to remain open at this time. This LPA requested that the facility representative go ahead and submit a letter detailing the decision to keep the license active at this time. There were no deficiencies observed or cited during today's case management visit. Exit Interview
