Pre-licensing visit·December 4, 2025

The LPA arrived unannounced on 12/4/2025, 9:30am, to conduct a prelicensing inspection. The LPA met with the licensee Kathrine Vilaneueva to explain the purpose of the visit. at the time of the visit there are renters in the home, the LPA will return for a second inspection in the future once the renters have left. Physical inspection has significant elements missing or to be improved: lockable storage for sharps and toxics and medication and records, night stands and lamps in the bedrooms, light lights on the way to the bathrooms, trash in the exteriors, extra bed linnen for all clients, 4 more beds, missing screen on the screen door. Required Posters(personal rights, ombudsman, federal workers rights, rights for resident councils) are not up. These elements should be resolved before the LPA returns. Administrator documentation, administrator certificate, Evacuation plan 610e were present and up to date, infection control plan should be reviewed by the administrator for completeness. Comp 3 presentation given, a copy of the report was read and given to staff. exit interview was conducted. facility is not ready for licensure at this time. a date will be calendered in feburary for revisit.
