Pre-licensing visit·February 26, 2026

Licensing Program Analyst (LPA) Albert Johnson conducted an unannounced pre- licensing inspection. LPA met with Yadao V. Facility was inspected both indoors and outdoors. LPA inspected bedrooms, bathrooms, kitchen, garage, and common areas. Outdoor exits are clear and accessible. There are two floors to this facility. The facility is an adult residential facility level four funded by Valley Mountain Regional Center with a current census of 3. Facility has 8 bedrooms and 4 bathrooms. Four bedrooms are for resident's use. Facility has a formal dining area and a formal living room. Hot water temperature was measured at 116.5 degrees. Smoke detectors and carbon monoxide detectors were checked and operational. Fire extinguisher indicator revealed a full charge. Kitchen is clean sanitary, and in good repair. There will is a locked areas for cleaning supplies, sharps, medications and toxins. Fire clearance was granted for 4 -ambulatory clients. Component III was waived. Licensee has been in service with the department for 23 years. The applicant has passed the pre-licensing inspection. LPA will notify the Central Applications Unit.
