What the inspector wrote

On 3/24/2023 at 9:00 am, Licensing Program Analysts (LPAs) Darlene Chavez and Mark Jeffries arrived at the facility to conduct an unannounced annual inspection. LPAs met with Hanileta Keohen, Licensee, and explained the reas on for the visit. During the facility tour, LPAs determined that Staff #1 (S1) was not associated to the facility and working with residents in care. At 9:07 am, LPAs confirmed S1’s identity. Licensee says that S1 was only working in the facility today. LPA Jeffries informed licensee that S1 must have a criminal background clearance prior to S1 working with residents in care. Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D): Civil penalties assessed in the amount of $100. Exit interview conducted, deficiency cited and the report, civil penalties, and appeal rights given to the licensee.
