What the inspector wrote

On 04/22/2025 at 08:16am Licensing Program Analyst (LPA) Haner-Tomasko arrived unannounced at the facility to investigate the allegations to this complaint and conduct an annual facility inspection. LPA met with Licensee Meynard Marcos and explained the purpose of the visit. During the visit, LPA interviewed staff, clients, licensee, and obtained relevant documents. On the allegations: Staff yell at residents and staff do not speak to residents with dignity and respect. It was alleged Staff 1 (S1) aggressively speaks to the residents, yelling at residents several times within a few minutes, and telling them repeatedly to sit down. Interviews revealed S1 will speak with a raised voice to residents and commandingly tell residents to sit down repeatedly. Based on the information obtained, the allegations are deemed Substantiated at this time. Exit interview, deficiencies cited on 9099-D, copy of report given, appeal rights given.

This requirement was not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above when S1 raised their voice commandingly at the residents and repeatedly commanded them to sit down.

    Based on record review, the licensee did not comply with the section cited above in 3 out of 5 staff who do have current 1st aid & CPR certification which poses a potential health, safety or personal rights risk to persons in care.

  • Central store medicines that create safety hazards

    Based on observation, the licensee did not comply with the section cited above in 1 out of 6 residents when LPA noted during facility tour unlocked over the counter medications in Resident 1's (R1's) private bathroom and R1's medical assessment states they are not able to safely manage their medications which posed a potential health, safety or personal rights risk to persons in care.
