Inspector’s narrative

What the inspector wrote

At 10:20am, on February 9th, 2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA called Licensee Amelita Aquino since no one answered the door. Licensee stated no residents have resided in the facility since it was licensed last year and they sent Analyn Estoque, the Licensee of a near by facility, to meet with the LPA and unlock the facility. LPA met with Analyn Estoque, announced who he was and the reason for the visit. Analyn and LPA conducted a full tour of the facility. This facility is a single story residential home with four resident bedrooms (two are dual occupancy) and three full bathrooms (one is en suite to resident room #4, one is a jack-and-jill bathroom connecting resident room #3 and the live-in staff bedroom and one is for public use). A live-in staff bedroom is located near the laundry room. There is a living room, family room, dining space, and kitchen. The family room has a self-contained fireplace for resident safety. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. The facility has battery operated smoke detectors in each room that are all working and the carbon monoxide detector is in the hallway and functioning normally. LPA observed a fire extinguisher near the kitchen that was in the green compression range. LPA noted that the facility has no obstructions in hallways, doorways or exits. There is a locking closet for medication storage near the laundry room. LPA conducted a staff record review of the Licensee/Administrator. LPA noted there are no residents or staff currently residing in the facility. At approximately 1:20pm the Licensee arrived at the facility. LPA and Licensee conducted a review of the annual care tool modules. There were no deficiencies cited at this time. Exit interview conducted, report signed, and report provided to the Licensee.
