In addition, when conducting medication count, LPA observed the amount of medications in residents' bottles did not correspond with the start date of the medication listed on the CSMR which poses an immediate health and safety risk to residents in care.

According to staff interviewed, they are unsure why R1's MAR had S1's initials for 10/20/25.
