What the inspector wrote

On October 27, 2025, San Bruno Regional Office conducted a non-compliance conference meeting with CEO, Carl Knepler, Senior Vice President of Operations, Beau Ayers, Administrator, Shayan Gheisar, Regional Director of Operations, Terry Rose and Director of Regulatory Compliance, Marlene Nelon. Present in the meeting was Licensing Program Managers (LPMs), April Cowan and Brenda Chan, and Licensing Program Analyst (LPA), Komal Curley. During non-compliance meeting, the following violations were discussed, 87464 Basic Services, 87465 Incidental Medical and Dental Care, 87463 Reappraisals, 1569.652 Termination of admission agreement upon death of resident; removal of resident’s property; refund of fees paid; notice of contract termination and refunds, 87303 Maintenance and Operation, 87506: Resident Records, 87355 Criminal Record, 1569.69 Employees assisting residents with self-administration of medication; training requirements, 87101 Care and Supervision, 87309 Storage Space, and 87411 Personnel Requirements. During this meeting, it was discussed, Community Care Licensing will increase frequency monitoring inspection visits to ensure compliance with this compliance plan of Title 22 regulation. The Licensee has agreed to contact the Technical Support Program. Licensee was provided the link below for resources and guidance to improve facility operations: https://www.cdss.ca.gov/inforesources/community-care/resource-guide-for-providers The Licensee was informed that additional civil penalties may be assessed, pending review. Report is reviewed with the Licensee and a copy is provided.
