Pre-licensing visit·February 20, 2026

On February 20, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Pre-licensing inspection. LPA met with administrator, Ethel Gumban and LPA explained the purpose of today's visit. The administrator provided a tour of the facility and LPA observed the indoor and the outdoor passageways are free of obstruction. This is a single-level facility consisting of 3 semi-private resident rooms (room #1, room #2 and room #4), 1 staff room (room #3), 2 full bathrooms, kitchen, living and dining room. The common areas are closed off by a self- closing door. There were no residents during the time of the inspection. LPA observed good lighting thought-out the facility and comfortable temperature is maintained. Bed sheets, linens, and towels were observed to be sufficient and able to meet the needs of the residents at this time. Fire extinguishers were last checked on 3/20/2025, smoke and carbon monoxide are operating properly. LPA observed postings including CCL complaint poster, resident's rights, Ombudsman, etc. The water temperature in the kitchen and the bath/shower rooms were measured at 107-118 degrees Fahrenheit. Comp III orientation is completed with the Administrator Pre-Licensing is now complete. Immediate Licensure is recommended pending final approval from the Central Applications Bureau.
