What the inspector wrote

On 10/31/2025, Licensing Program Analyst(LPA) John Calandra, arrived at the facility to conduct the Pre-Licensing inspection. LPA Calandra was greeted by Monica Mendoza Ceron, Executive Director and explained the purpose of the visit. LPA Calandra toured the physical plant. This is a 5-story building with 108 bedrooms and bathrooms, a library, kitchen, dining room, lobby, and offices. Per interview with the Executive Director, the facility's fire alarm system is directly connected to the fire department. The facility's fire alarm panel was observed to be in working order. All bedrooms had the required furniture and sufficient lighting. The facility was maintained at a comfortable temperature. The facility had the required 7 days of non perishables and 2 days of perishables on hand. No food was expired. All sharp objects, soap, detergents, and poisons were observed to be locked and in-accessible to persons in care. LPA reviewed 5 resident files and 6 staff files. All were observed to be complete. A review of Centrally Stored Medications indicated that medications for residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication Records(CSMR) kept at the facility. LPA requested the following documents be sent to the Department by 11/7/2025: Admissions Agreement No deficiencies were cited during today's visit. Pre-Licensing is now complete. Immediate Licensure is recommended pending final approval from the Central Applications Bureau. An exit interview was conducted. This report was reviewed with facility representatives and a copy provided via email.
