Applicant Almacare Inc. has applied for RCFE licensure to care for 12 non-ambulatory elderly residents. Fire clearance has been approved. Facility is currently operating and licensed as Heritage Inn #415600558. LPA Audrey Jeung inspected building and grounds, seven-day emergency food supply, dishes, utensils, equipment necessary for the storage, preparation and service of food, linen supply, checked hot water temperature in main client bathroom at 110 degrees. PPE and first-aid kit are present. Facility floor plan is accurately reflected on facility sketch, including locations of utility shut offs. Washer and dryer are located in front hallway near kitchen. A Smart Caregiver emergency signal system is tested and operable, consisting of pendant alarms which are connected to main unit in office. An auditory alert is activated that identifies each resident. Items referenced in Emergency Disaster Plan are reviewed and verified. This facility is essentially one level, with seven client bedrooms, staff room, 5 full bathrooms, a half bathroom, office, full kitchen, living room, dining room, and sun room. There is a detached garage, which is used by licensees, and not currently used by facility staff. The rear yard is shared with adjacent RCFE, Heritage Park, also owned by this licensee. It is level and fenced. A locked storage shed is located in the backyard, and contains building supplies. A dedicated wrench for shutting off the main gas line is recommended to be readily accessible. There is locked storage space designated for medications and toxic cleaning products in the office. Hospice care plans for 2 hospice residents and Centrally Stored Medications Records for all residents are maintained. Staff do not handle clients' money. Facility phone number is 650/348-5585. Component III orientation is waived, as applicant Almacare Inc. currently operates 4 other RCFEs in substantial compliance with Title 22 requlations. Facility meets physical plant requirements for RCFE requirements per Title 22 of California Code of Regulation. Immediate licensure is recommended, pending final approval by Centralized Applications Branch.
