What the inspector wrote

Licensing Program Analyst (LPA) Kristin Kontilis conducted a Case Management visit to address deficiencies noted during Complaint Control #29-AS-20230512135542 investigation visit conducted on 5/17/2023. LPA met with Mark Cortes and explained the purpose of the visit. LPA reviewed Department of Social Services, Community Care Licensing Division, Licensing Information System (LIS), Facility Personnel Report summary and determined that Staff 1 (S1) ended employment in the facility on 12/13/2022, then returned to work in the facility on 1/16/2023; and was not re-associated to the facility prior to returning to work on 1/16/2023. Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D): Civil penalties assessed. Exit interview conducted. Copy of report, Appeal Rights, and civil penalties issued at the time of the visit.
