What the inspector wrote

Licensing Program Analyst (LPA) Kristin Kontilis conducted a Case Management visit to address deficiencies noted during Complaint Control #29-AS-20221201151546 investigation visit conducted on 12/5/2022. LPA met with Medication Technician Ana Delgado and explained the purpose of the visit. Administrator Miriam Santiago was unavailable at the time of the visit. On or about 11/29/2022, CCL received information from Credible Witness 1 (CW1) stating during the early hours of 11/26/2022, AMS Emergency Responders and Santa Barbara Fire Department responded to a 9-1-1 call to the facility. CW1 reported First Responders noted several incidents in which “multiple” residents were taken to the hospital due to health and safety issues and concerns for their health. As of today’s visit, no incidents of serious illness or injury reports for five residents on 11/26/2022 have been received by CCL for Resident 1 (R1), Resident 2 (R2), Resident 3 (R3), Resident 4 (R4) and/or Resident 5 (R5). Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiency was cited (refer to LIC 809-D): Exit interview conducted. Copy of report and Appeal Rights issued via email.
