What the inspector wrote

Licensing Program Analyst (LPA) Olson conducted an unannounced case management visit to issue a deficiency. LPA Olson received multiple Incident Reports (IR) from the facility that were sent after the required 7-day time frame. LPA provided courtesy reminder emails to the Administrator on 3/30/2022, and to the Administrator and Wellness Director on 5/27/2022. LPA continued to receive Incident Reports late, including: · On 6/7/2022, LPA received IRs from 5/22/2022 (16 days after incident), and from 5/18/2022 (14 days after incident). · On 6/23/2022, LPA received IRs from 6/15/2022 (8 days after incident), 6/14/2022 (9 days after incident), 6/13/2022 (10 days after incident). · On 6/29/2022 LPA received IRs from 6/19/2022 (10 days after incident), 6/18/2022 (9 days after incident) and 6/17/2022 (8 days after incident). Based on the information obtained, the facility is cited for late reporting. Exit interview, report given, deficiency cited on 809-D, appeal rights given.

    87464(d) Basic Services. …If a facility chooses to accept a particular resident for care, the facility shall be responsible for meeting the resident's needs as identified in the pre-admission appraisal...either directly or through outside resources.This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the above cited section when R1 did not receive a shower for 22 days after moving into the facility, which posed a potential health and safety risk to residents in care.

A written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of the occurrence of any of the events specified in (A) through (D) below...This requirement was not met as evidenced by: Based on record review, the licensee did not ensure incident reports were sent within 7 days of the occurrence on multiple occasions, which posed a potential health, safety, and personal rights risk to residents in care.

    87625(b)(3) Managed Incontinence...the licensee shall be responsible for the following: Ensuring that incontinent residents are kept clean and dry and that the facility remains free of odors from incontinence.This requirement is not met as evidenced by: Based on record review, interviews and photographs, the licensee did not ensure residents’ incontinence was properly managed, which poses an immediate health and safety risk to residents in care.
