This requirement was not met as evidenced by; Based on interviews conducted, and evidenced reviewed, staff S1, S2 and ADM admitted that resident R1 feet were tied because he/she was being combative when he/she was being changed. This poses an immediate threat to residents health, safety and personal rights.
