During review of 5 out of 5 resident Centrally Stored Medication and Destruction Records (CSMDRs), Licensee did not ensure that 2 out of 5 residents had medications which were not completely recorded (R1 had one medication missing a prescription number and R2 had four missing medications), which poses a potential health risk to residents in care.

87211 Reporting Requirements. (a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, (1) A written report shall be submitted to the licensing agency ... within seven days of the occurrence ...(D)Any incident which threatens the welfare, safety or health of any resident ... The requirement was not met as evidenced by:Based on the records reviewed, Administrator did not send the incident report of resident R1 was sent to hospital within 7 days of the incident occurrence, this poses a potential health, safety or personal rights risk to persons in care.

Keep window screens clean and in repair

Based on observation, 3 window screens were observed not in good repair, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on observation, the cabinet under the sink in kitchen was observed unlocked, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Call 9-1-1 for imminent health threats

87465 Incidental Medical and Dental Care (g) The licensee shall immediately telephone 9-1-1 if an injury or other circumstance has resulted in an imminent threat to a resident’s health.This requirement was not met by: Based on interviews and records review, on 5/14/2021, staff did not seek timely medical services to resident who obtained severe injuries in the facility. This posed an immediate risk to the health, safety of the resident in care.
