Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Designated Administrator (ADM) Lilian Sacun. 2 staff and 5 residents were observed in the facility LPA checked 3 resident files and 3 staff files. LPA toured the facility inside out with ADM. Family room, kitchen, dinning room and 2 restrooms were inspected. 5 resident rooms, and laundry area were inspected. One staff live-in room was observed in the facility. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. Medication closet was observed locked. Knives closet and cleaning product closet were observed unlocked. Staff locked knives closet and cleaning product closet immediately. The Room temperature was at 74 degree F, and hot water temperature was at 118 degree F in facility. Fire extinguisher was serviced on 9/2/2025. First aid box, night lights, and flash lights were observed in the facility. The last time the facility conducted the fire alarm drill was on 1/5/26. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Smoke detector was found out of battery. Staff replaced the battery immediately. Front yard and backyard were inspected. There was no obstruction to block the walkways. . No citation noted today. Exit interview was conducted with ADM. This report was provided to ADM for signature. A copy of the report was provided to ADM.
