Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced case management-Incident visit and met with General Manager (GM) Ida Gemignani-Stearns and Senior Resident Care Director (SRCD) Michael Lucio regarding an Incident Report (IR) submitted to the Department on 6/30/2025. LPA stated the purpose of the visit. On 6/30/2025 the Department received an IR for Resident R1 who reported financial theft on 6/26/2025 by unknown individuals. During today's visit LPA interviewed GM, SRCD and 1 resident. LPA requested additional documentation and an SOC 341 to be submitted to the Department by 5PM on 7/2/2025. No deficiencies cited during today's visit. An exit interview was conducted with SRCD Michael Lucio, and a copy of this report was provided.
