Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct a case management – incident visit. LPA met with General Manager, Ida Gemignani-Stearns. The purpose of the visit is to follow-up on a death report that Department received on 08/12/2025 regarding a former resident (R1). During visit, documents were obtained to include R1’s face sheet, service plan dated 02/18/2025, progress notes, and staff schedule from August 2025. A witness and 2 staff members were interviewed. This case management will be kept open pending further investigation. This report was reviewed with General Manager, Ida Gemignani-Stearns and a copy of the report was emailed to the GM due to technical difficulties.
