What the inspector wrote

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with the house manager (HM/STAFF) Mary Grace Yongco and stated the purpose of the visit. Administrator Arielle Teodoro was not available at the time of inspection due to prior commitment. ADM authorized HM/STAFF to sign for the report on ADMs behalf. The facility is licensed for adults 60+; approved for 6 ambulatory only residents. LPA observed 3 out of 6 residents waiting for their ride to go to the day program 2 out of 6 have residents are have been picked-up and 1 out of 6 was in his room does not attend day program. 1 staff was present during the visit LPA toured the facility, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor spaces. Indoor temperature was within acceptable range of 70°F. The kitchen was clean and organized; knives and chemicals were locked. Food supply met requirements (2 days perishable, 7 days non-perishable). Kitchen water temperature measured at 115.1°F. Bathroom water temperature ranged from 109.7°F to 113.5°F. Bathrooms had grab bars and non-skid mats; resident rooms had adequate storage. LPA inspected 5 resident bedroom 4 out of 5 were single occupancy and 1 out of 5 is shared. Medications were locked and inaccessible to residents; first aid kit was complete. Outdoor areas were free of hazards; laundry appliances were functional, and cleaning supplies were secured. Fire, smoke, and carbon monoxide systems were operational; hallways were clear and well-lit. Page 1 of 2 The facility has storage area at the back that is used to store unused items and files that are no longer in use. The facility has an aluminum ramp at the back of the property. 3 cars were observed parked on the driveway and a side exit door inside one of the resident bedroom was also observed. LPA reviewed 3 resident and 3 staff records, including medication logs, admission agreements, care plans, personal and incidentals, health screenings, and training. All staff have required clearances and certifications. The facility conducts fire and earthquake drill quarterly for each shift. Last drill practice training was on 01/07/26 with 2 staff and 6 resident participation. The facility is equipped with fire panel alarm system and 1 fire extinguisher that was inspected 01/16/25 and fire sprinkler. No deficiencies were cited during today's visit based on the California Code of Regulations (CCR) Title 22. An exit interview was conducted with HM/Staff Mary Grace Yongco and a copy of the report was provided. page 2 of 2 end of report
