Pre-licensing visit·September 12, 2025

Licensing Program Analyst (LPA) David Marrufo conducted a Prelicensing visit and met with Administrator Justin Ladwig. During visit, LPA toured the facility inside and out. LPA toured the kitchen area, which had a lock box for sharp objects and two available refrigerators. LPA Marrufo observed the locked office room that included shelves for resident and staff records and a storage area for cleaning supplies. LPA observed the first aid kit and observed it to be complete. LPA observed 6 out of 6 resident bedrooms. Each bedroom had beds, working lighting, and available clothing storage areas. LPA toured 4 out of 4 bathrooms. Each bathroom had working lights, grab bars, anti-slip mats, and available soap and paper towels. The water temperatures in the bathroom sinks measured from 116 F to 119 F. During visit, the smoke and carbon monoxide detector system was tested and found to be functioning properly. LPA toured the outside area and observed the outdoor exits to be clear of obstructions. During visit, LPA Marrufo reviewed the Component III Orientation with Administrator. Pre-Licensing is complete and this facility has no deficiencies. This report was reviewed with Administrator Justin Ladwig and a copy of this report was provided.
