What the inspector wrote

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a follow up prelicensing visit for deficiencies observed on 12/15/2025. LPA met with Applicant Josephine Guillermo. There are 2 residents present on site. This facility address is currently licensed (43520259) and is under a change of ownership (CHOW). On 12/15/2025 during a pre-licensing visit deficiencies were observed and documented in the Pre-Licensing report. LPA observed the facility to have damaged flooring in the kitchen near the sink, a damaged window sill in Bedroom #4, and damaged closet door tracks and blinds in Bedrooms #4 and 5. Bedroom #5 also had a damaged window screen. In addition, LPA observed 4 loose wooden handrails outside the kitchen sliding door and outside Bedroom #1, and the kitchen screen door to have a large hole near the screen handle. The current licensee needs to address that damaged areas and ensure there are no damaged, loose or broken areas prior to issuing license to new owner. On 12/26/2025 LPA inspected the facility and observed the deficiencies that were observed on 12/15/2025 to be corrected and all damaged areas documented were observed to be in good repair. Component III orientation was completed with Licensee. No issues noted during this pre-licensing inspection. LPA observed the facility is ready to be licensed. However, this report will be submitted to the Central Application Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required. This report was reviewed with Applicant Josephine Guillermo and a copy of the report was provided.
