What the inspector wrote

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Administrator Janet Leon. The purpose of the visit was to issue a deficiency citation to the facility for advertising dementia care services without having a facility dementia care plan in the facility program design. LPA Marrufo reviewed the facility file on 04/25/2025 and observed that the facility did not have a dementia care plan in its program design. The program design's "Prior to Admission" section states, Will accept ambulatory and non-ambulatory, diet problems, diabetic, arthritis, and general aging.” The facility website states, "Accommodation: Secure Living Program - Dementia / Alzheimer's Care" During visit, LPA Marrufo obtained copies of Physician's Reports for residents R1-R2, whose Physician's Reports stated they had a diagnosis of dementia. A deficiency was cited as per California Code of Regulations Title 22. See LIC809-D page for more information. This report was reviewed with Administrator Janet Leon and a copy of this report and appeal rights were provided.

This requirement was not met as evidenced by: Licensee did not ensure that while the licensee advertised dementia care services on the facility website that there was also a Dementia Care Plan in the facility Plan of Operations, which poses a potential health and safety risk to residents in care.
