What the inspector wrote

Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Case Management visit to follow up on the facility's capacity increase request. LPA met with Licensee Jesus Arcilla and Administrator (ADM) Minerva Velasco. LPA stated the purpose of the visit. On 07/18/2025 the Department received a capacity increase request from Licensee Josephine Arcilla. Licensee requested an increase in capacity from eight (8) to thirty-two (32) on 7/14/2025. This facility has 2 separate buildings, Building #1 and Building #2, which share the same address. Licensee requested 1 ambulatory, and 31 bedridden, with a total capacity of 32. A Fire Clearance request was submitted to the local fire department on 8/26/2025. On 12/29/2025 the Department received the fire clearance approval: Building 1-Rooms 1-4, and 6 approved for bedridden or non-ambulatory residents, Room 5-approved for ambulatory ONLY. Building 2-Rooms 1-12 approved for bedridden or non-ambulatory. During today's visit, LPA inspected 6 bedrooms in Building 1, which currently has 5 residents, all exit doors/sliding glass doors are equipped with auditory alarms. LPA inspected Building #2, to include all 12 bedrooms. All 12 bedrooms had a bed, dresser, chair, and space for personal belongings and functioning lights. All 12 bedroom exit doors/sliding doors are equipped with auditory alarms. All 12 bedroom auditory alarms functioned properly when tested by the Licensee and ADM. Building 2 does not currently have any residents. Licensee and ADM were advised the facility cannot accept residents until the Department has processed all documentation for the capacity increase. Licensee and ADM stated understanding. No deficiencies cited during today's visit per California Code of Regulations, Title 22. An exit interview was conducted with ADM Minerva Velasco and a copy of this report was provided.
