What the inspector wrote

LPA attempted staff interviews multiple times with no success. Document review revealed, on 3-24-25 at 7:30PM staff reported R1’s 7:00AM medication was still in medication box and not given. Document review revealed on 3-25-25 R1’s 8:00PM medication was given at 8:00 AM. Document review revealed R1’s 8:00AM medication was given at 1:00PM. Document review revealed no incident reports were submitted to Licensing concerning medication errors in March 2025. Based on investigation observations, record review(s) and interviews which were conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated . California Code of Regulations, (Title 22), is cited on the attached LIC 9099D. Appeal rights were explained and provided to the facility representative listed above and exit interview conducted. If any of the cited deficiencies are not corrected by the noted due date, civil penalties may be assessed.

This poses a potential health and safety risk to the residents in care.
