On July 29, 2024 at approximately 11:30 AM, Licensing Program Analysts (LPAs), Farhaan Sarangi and Sarah Benson arrived unannounced for the purpose of conducting a Case Management-Other inspection. Upon arrival, LPAs were greeted at the door by Caregiver, Rebekah Alvarez and was granted access into the facility. Administrator, Stephen Lawson arrived 30 minutes later. During the Case Management-Other inspection, LPAs reviewed the Guardian Background Clearance List and observed that the Caregiver is not associated to the facility. The Caregiver is background cleared but was never associated to the facility. LPA educated the Licensee and the Caregiver regarding the importance of transferring ALL individuals that have been Eligible/Cleared to work in the facility (See LIC 9102-Technical Violation). No deficiencies were cited during today's Case Management-Other inspection. Exit interview was conducted and a copy of this report was emailed to the Licensee.
