It was determined there has been lack of staffing at times. Based on (title 22) When regular staff members are absent, there shall be coverage by personnel with qualifications adequate to perform the assigned tasks. Based on investigation observations, record review(s) and interviews which were conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated .
