What the inspector wrote

Licensing Program Analyst (LPA) Nakagawa arrived unannounced and met with Juliet McGranahan, Administrator to open this complaint investigation and deliver findings. During the course of this investigation the facility was toured, records and photos were reviewed and interviews conducted. It is alleged that Licensee did not ensure staff followed proper infection control protocols. During the course of the investigation, photographs were reviewed and interviews were conducted showing that used PPE was not properly doffed by staff; nor was it properly discarded, stored or removed during an active case of Covid-19, therefore this allegation is substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Deficiency cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiencies and/or repeat deficiencies within a 12 month period may result in civil penalties. Appeal rights given.
